Description:
The primary purpose of your job position is to provide safe and timely Transportation and Logistics of PACE participants, including
transport meals, durable medical equipment, pharmacy, and any other Transportation and Logistics as deemed necessary.
Responsible for overall cleanliness, safety of the vehicle assigned on a daily basis, and communicating any safety issues
to the department leader.

We are looking for compassionate and caring Driver to join our LIFE St Mary team. LIFE St Mary Healthcare is a member of Trinity Health PACE. Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period.

This position will be housed primarily at the LIFE St Mar facility. However, occasional home care work may be required.

What you will do:

Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
Assist participants in the maintenance of physical activity, exercise, and walking
Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.


What you will need:

Must possess a comprehensive knowledge obtained through high school diploma or equivalent. Associate's degree preferred.









Valid driver's license in good standing required. CDL license with passenger endorsement required to operate company vehicles that exceed 15-passengers.
Minimum one (1) year of experience working with the frail or elderly population.
LIFE St Mary Drivers û Active CPR certification required.
