Clinical Case Manager
Incorporated in 1970, Visions and Pathways (V&P) was New JerseyÆs first private 501(c)(3) non-profit youth shelter. Our mission is to provide abused, neglected, homeless and missing youth with housing, a stable environment and supportive services that help them become self-sufficient.
We are seeking a full time Clinical Case Manager. The Clinical Case Manager is part of our residential services team, reporting to Visions and PathwaysÆ Director of Residential Services.
Essential Duties and Responsibilities
Provide maintain case management case load in accordance with the guidelines set forth in the case manager job description, per the following:
À Ensure that all information is up-to-date Perform Care for payment and other purposes.
À Conduct new client intakes.
À Coordinate services for residents according to treatment plan (therapy, psychiatry, medical, DCP&P, family contacts, UCM, etc.).
À Keep files up to date with treatment information.
À Maintain and document a minimum of two contacts weekly with residents.
À Maintain and document a minimum of one family contact bi-weekly.
À Develop client treatment plans (bi-weekly plans for shelter residents).
À Crisis counseling
À Attend court hearings, IEP meetings, initial psychiatric appointments, and other community meetings as they relate to specific cases.
À Maintain case records and ensure compliance with licensing standards.
À Help ensure life skills education services are provided and completion of Ansell-Casey life skills assessment, AIF, RHYMIS, HYMIS, billing reports, LOS and case management reports monthly and quarterly.
À Maintain statistical data regarding youth and prepare year end stats.
À Follow guidelines as outlined in Rules for Child Care Workers and all program manual policies and procedures.
À Ensure that discharge planning occurs and is well document.
À Coordinate activities and services with Director of Residential Services to ensure comprehensive service delivery to agency clients.
À Ensure completion of psycho social/substance abuse assessments on all youth by agency Therapist.
À Participate in Case Management meetings, treatment team meetings, and Quarterly Treatment Plans (QTP).
À Share on-call responsibilities.
Qualifications and Experience:
Clinical Social Worker, MasterÆs Degree in Social Work, plus five years of experience.
Excellent organizational, computer and communication skills.
A valid NJ driverÆs license and a minimum of three years driving experience.
English/Spanish bilingual a-plus.
